    We have been doing school immunization clinics for a while and have been using paper forms for registration. We would like to move to online registration for future clinics. Does anyone have suggestions for online clinic registration products? We looked at Vaxcare, but they don’t allow for parents to select the vaccines they are consenting for. We are wondering if anyone has had something they really like.

    We utilized JotForm with our school clinics exclusively this year. It has the capability to allow photo upload of insurance card, digital signature, selection of which vaccines, screening questionnaire, etc. You create your own consent form. Our agency upgraded our account with their service to be HIPAA compliant and encrypt forms as well.

    I do believe that we had less enrollment of children with the digital form due to many of our students having guardians who may not utilize electronic forms. The school was very helpful and submitted an email out to all parents with a QR code that would take them straight to the form. I found it to be a great tool and less cumbersome than the traditional paper consents.
